First Visit Appointment

First Visit appointment: please bring with you insurance information, the insurance booklet you received with your insurance package, list of all your medications; please arrive 10 minutes prior to the appointment to complete the your chart profile.

Re-Schedule or Cancel Appointment

Please call us with at least 24 hour notice of canceling or rescheduling of your appointment so we could offer that time slot for other patients in need.

Methods of Payment, Financing and Insurance Providers

All methods of payment accepted (Visa, Mastercard, Interac/Debit, cash and cheque). Financing can also be arranged; please see Affirm link below.

We offer direct billing (with all insurance providers that accept it) for most procedures.

The loyalty of our patients is of utmost importance and appreciation. We welcome new patients.

If there is a balance owing, the payment is required immediately following the appointment.

For some treatments and procedures, an insurance pre-approval or pre-authorization is required. We will send information required on your behalf.
